Vaccinated persons can obtain the certificate required to travel abroad at a medical facility providing immunization services. The vaccination provider shall certify the document with the seal and/or stamp of the same institution and the signature of the authorized person.
The COVID-19 vaccine certificate includes the following information:
- Personal data of the vaccinated person: name, surname, personal number, date of birth;
- Vaccine details: dose, manufacturer, date;
- Name of the medical institution;
- Information on the results of the COVID-19 testing and the research laboratory.
A vaccinated person can request a certificate from the National Center for Disease Control and Public Health, which also certifies the document with a stamp and/or stamp and the signature of an authorized person.
Upon request of the receiving country, this certificate may be certified by apostille by the Public Service Development Agency.
The procedure of apostille certification of vaccine is a temporary measure before the accession of the QR codes of the documents issued by Georgia to the EU area.
